Application Guidelines
Parents of new and returning students are required to complete an application package for each child for the upcoming school year. The application package consists of several forms that must be completely filled out, dated and signed by parents or legal guardians prior to the student’s first day of school. Please contact AFA office for registration and enrollment process.
The following documents are required for all Preschool, VPK and Kindergarten students:
1) Birth certificate
2) Current Immunization & Physical Health Reports
3) AFA Registration Form
